White City VA Expanding Who Can Get the Covid-19 Vaccine

Southern Oregon veterans who are 65 years and older can now get the Covid-19 vaccine from the VA center in White City. You must be 65 and older, a Veteran, and you must already be “enrolled” for VA Healthcare.in the state of Oregon. They are administering the Moderna vaccine

If you are not already enrolled in VA Healthcare you can complete and submit the application online, or if you are more comfortable printing the form and mailing it in, here is a link to the Fillable, printable form which you will then mail with supporting materials to the Health Eligibility Center, 2957 Clairmont Road, Suite 200, Atlanta, GA 30329
