Parents, a new school year begins in September 2015, and with it, Religious Explorations will begin a new program year. We will need you to review and update information on-file for your child.

A medical information form is part of a safe congregation manual policy, adopted by the RVUUF Board on June 11, 2015. Parents, you wil be asked to fill out this form as part of registering/updating your child’s information.

Please also review your contact information and your permissions on-file for the use of photos and video.

The forms can be accessed as PDFs from Religious Explorations’ page on the RVUUF website (https://rvuuf.org/worship/childrens-program/) as well as from the “Files” tab at RVUUFian Parents on Facebook (https://www.facebook.com/groups/RVUUFian.Parents/).
